What you’ll do

We will look to you to support the social services team by planning, organizing, implementing and evaluating social services for residents and patients within our life plan communities. You may complete admission contracts and discuss insurance overage with residents, help with discharge planning, or even orient them to the campus. You will provide patients and families with the support and care the need to ensure their needs are met.

 

What it takes

  • High school diploma or equivalent required
  • Associate degree in a related field preferred
  • Two years’ experience in a healthcare and/or human services environment required
  • Experience in patient advocacy, a long-term care facility, hospital, or other related healthcare setting preferred
